$100.14 per hour - this Support Item is to help strengthen a persons, or Participants ability design and then build their supports with an emphasis on linking the broader systems of support across a complex service delivery environment. This Support Item is to focus on supporting people to direct their lives, not just their services, involves resolving points of crisis and developing resilience in the persons network. (ie. knowing who you can talk to when you have a problem).
$190.54 - This This support is delivered utilising an expert or specialist approach, necessitated by specific high complex needs or high-level risks in a participant’s situation. Specialist Support Coordination is delivered by an appropriately qualified and experienced practitioner to meet the individual needs of the participant’s circumstances such as a Psychologist, Occupational Therapist, Social Worker, or Mental Health Nurse. Specialist Support Coordination is expected to address complex barriers impacting a participant’s ability to implement their plan and access appropriate supports. Specialist Support Coordinators assist participants to reduce complexity in their support environment and overcome barriers to connecting with broader systems of supports as well as funded supports.
Not everyone can speak, read or write; for some they may not be able to speak English or maybe you cannot hear and use Auslan to communicate. My service is to support you, where you are at.

Within my role there is a fair bit of paperwork and I do acknowledge that for a lot of people reading and or understanding this can be quite stressful.
My Aim is to work with you, so please let me know how best I can support you in this area.
Under the NDIS quality and Safeguards Standards, there is a lot of reporting I am required to do, this is to ensure:
- I am complying with my NDIS obligations
- you are receiving the right services for you
- you are living a life of choice, control and free of Abuse, Neglect or Exploitation.
The reason for this is sadly, historically, people with a disability have often been the victims of Abuse in all of it's different forms.
My role is to help ensure you are safe.
I am able to meet with you in a variety of community venues close to your home. There are many different locations to choose from which have great disability access and parking. Before the first time I meet with you I will be going through a Risk Assessment, which is standard for all people I meet with.
I do understand that for a variety of reasons you may have to have your support meetings in your home, to do this safely, I contract SHEQSY Security Services to monitor my location.
A Service Agreement is like a contract, it is something we develop together, where both you and I understand what our rights and obligations are.
The Action Plan is like a service Plan, which you may of used before with other services. The Action Plan is a written record of what your NDIS Goals are, how you want to achieve them and who you want to connect with to achieve these goals.
My role is to give you information, give you pro's and con's of your choices and support you in your decisions.
If at any time a decision you want to make is against the law, could hurt you or hurt others, then I will be unable to support you with that decision and depending on the circumstances also have to report the situation.
A big part of my services is collecting regular feedback forms from you.
These forms provide the basis on shaping my business, improving my services and more importantly giving you regular formal opportunities to provide feedback to me.

In providing Support Connection and Specialist Support Coordination to people, I am required to complete various reports of your progress to your NDIS Planner.
My aim is to always involve you in the report planning and writing.
I personally value transparency and like to model it in the services I provide to people. This means that I am directed by you with your services and I like using your words of how you are finding the services you use and how they are helping you to reach your NDIS goals.
So upon entering a Service Agreement with me, I'll be outlining the Reporting your NDIS Planner wishes me to undertake and incorporating this into your Support Hours. For everyone this could be slightly different. Generally though there is an 8 week report, where the NDIS wants to see that most of services you have funding for has started and any problems with supports are worked through; then there is a end of funding report for the end of your Support hours.
Sometimes NDIS Planners request extra reporting.
Depending on your disability and circumstances, the NDIS understands that at times ongoing Support Coordination hours may be required, if so, part of the reporting will include how and in what way this will be needed.
